Compassionate grief support for families after loss

About This Service

Grief does not end when a loved one passes. Our bereavement services extend support to families for up to 13 months following a loss, providing counseling resources, check-in calls, and connection to community grief support. We believe that caring for families is part of our mission — even after our direct care has ended.

Who It Helps

Bereavement services are available to family members and loved ones of patients who received hospice care through Ability HHH.

What to Expect

Bereavement support may include follow-up calls from our care team, written resources on grief and healing, referrals to grief support groups, and access to our social workers and chaplains.

Care Coordination

Our bereavement coordinator works with families to provide appropriate support and connect them with community resources as needed.
